PD Funding
Submission Guide

This step-by-step guide explains how to correctly send your PD Funding Form and Certificate to the Government of Alberta to receive your funding.

Before You Start
Download and Complete Your PD Funding Form

Before sending your PD Funding documents, you must complete the PD Funding Request Form.

Step 1

Employment & Personal Information

​

  1. Child Care Program Name: write the official name of your child-care program.

  2. Program ID Number: ask your Director/Owner for this number — you must include it.

  3. Last Name & First Name: your full legal name.

  4. Certification Level: select your ECE level (1, 2 or 3).

  5. Certificate Number: the number from your ECE certificate.

  6. Status in Canada: choose one option (Citizen, Permanent Resident, Work Permit or Study Permit).

    • If you are on a Work or Study Permit, you must include the permit number and expiry dates, and later attach a copy of your valid permit to your email.

Step 2

Workshop
Details

​

  1. Check ✅ “Workshop” (not Conference or Flight).

  2. Name of Company Presenting Workshop: Churcher Group Ltd.

  3. Name of Workshop: use the exact title from your certificate (e.g. “Classroom Excellence – Turn Regulations into Practice”).

  4. Workshop Date: the date shown on your certificate.

  5. Date Paid & Amount Paid:

    1. If you paid for the workshop → enter the date and amount.

    2. If it was free → enter $0.00.

  6. Request the Release Time Grant: mark ✅ Yes.

Step 3

Declaration
& Submission

​

  1. Check all boxes confirming that you:

    1. informed your employer,

    2. attached your Work/Study Permit (if applicable),

    3. attached your Certificate of Completion.

  2. Enter your full name, phone number, email, and date.

    1. If your director submits on your behalf, their contact info goes here instead. 

 

Save the completed form as PDF before sending it by email.

How to Submit Your PD Funding Documents

Step 1

Before You Start

Make sure you have the following ready:

   âœ… Your PD Funding Request Form (filled and signed)

   âœ… Your Certificate of Completion (from your workshop)
   âœ… (If applicable) Your Study Permit or Work Permit if you’re not a Canadian citizen or permanent resident.

 

Tip: Save all files as PDFs and name them clearly (e.g. “PD_Funding_YourName.pdf”).

Step 2

Open your email

Open Gmail, Outlook or your preferred email platform.

Step 3

Attach your files

Attach:

  • PD Funding Request Form (filled and signed)

  • Certificate of Completion (from your workshop)

  • (If applicable) Study/Work Permit

Step 4

Write your message

You can copy this example into the email body:

Hello,
Please find attached the PD Funding Request Form and Certificate for [Your Full Name] from [Centre Name].
Let me know if anything else is required.
